Abstract: Elderly is a stage of a life process marked by decline, weakness and vulnerability of the body to disease and the environment. The main problem in the elderly is the emergence of several chronic diseases so that a chronic disease management program (Prolanis) was created to improve the quality of life and maintain the health of its members. The event was attended by representatives of prolanis members as many as 30 participants while maintaining health protocols. The evaluation was carried out by filling out a questionnaire where there was an increase from the most participants during the pre-test being able to answer 75% of the questions correctly and increasing to the most participants during the post-test being able to answer 80% of the questions correctly.
