Abstract: Delays in maternal referral can lead to maternal mortality. A 36-week pregnant woman with eclampsia died due to a lack of knowledge among the mother, family, and community in recognizing early signs, maternal risks, complications, and the inability to make referral decisions or contact healthcare professionals. This community service aims to develop self-sufficient community organizations to avoid maternal referral delays. The activities are divided into three stages: Pretesting, counseling, and showing the use of early detection monitoring sheets are all part of Stage 1. Stage 2 directs and supports the target partners in their homes using monitoring sheets. The activities are evaluated in Stage 3 with the participation of target partners, followed by a post-test. Community leaders, community health workers, co-residing family members, and 40 pregnant women are among the project’s collaborators. The results demonstrate a 53% increase in knowledge and a 100% ability to monitor early identification of maternal emergencies utilizing the monitoring sheets.
